The Ostomy Skin Tool: A peristomal skin assessment tool
This is a standardised way to
- Score the peristomal skin through objective observations
- Categorise the peristomal skin disorder according to their cause
What is the tool meant to do?
- The tool is designed to assess peristomal skin
- The tool is designed to assess the state of the peristomal skin at that particular point in time
- Assess if there has been an improvement or a degradation over time
The Coloplast Ostomy Skin Tool consists of two parts:
- The DET score (D for discolouration, E for erosion, T for tissue overgrowth)The total DET score gives information on the overall severity of the peristomal skin problem, whereas the subscore helps to define the problem.
- The diagnostic guide is used to establish which are the most relevant visual changes. Following the flow of the guide, you consider a set of questions appropriate to the observations. Finally, the disorder can be categorised according to its cause and a treatment regimen decided.
Important to remember: Both DET score and diagnostic guide should be used every time a peristomal skin area is assessed.
